Actos is a type 2 diabetes medication containing the active ingredient pioglitazone hydrochlorite.  Pioglitazone is classed as a thiazolidinedion or glitazone medication and is used to help to control blood sugar levels in non-insulin dependant diabetics.

How does it work?

In people without diabetes, the pancreas produces a hormone called insulin which is responsible for the control of blood sugar.  The pancreas does not produce enough insulin in type 2 diabetes and their body cells are also resistant to the low levels of insulin that are in the blood.  As insulin normally tells the cells to take sugar from the blood, in type 2 diabetics, the blood sugar levels rise too high.

Actos increases the sensitivity of the liver and the fat and muscles to insulin and by so doing lowers the blood sugar levels.  The cells are then able to remove glucose from the blood more efficiently.  Actos also cuts the amount of glucose the liver produces and protects the functioning of the beta cells in the pancreas that produce insulin.

Uses

Actos is used to treat patients with type 2 diabetes.  It is particularly good for patients who are overweight that cannot control their blood sugar levels enough by diet and exercise.  It is also used by people who cannot take metformin.

When combined with metformin, or a sulphonylurea diabetes medication or both of these, Actos is used by patients whose blood sugar levels are not controlled by the maximum doses of these other medications.  Actos can also be taken combined with insulin by patients with type 2 diabetes whose blood sugar isn't controlled enough by insulin and can't take metformin.

Warnings

Actos can cause the patient to gain weight so they must stick to a calorie controlled diet and closely monitor their weight.  Actos can sometimes, but rarely, cause fluid retention that can lead to heart failure so the patient must consult their doctor immediately if they suffer from shortness of breath, swollen ankles or excessive and rapid weight gain.

The patient should let their doctor know if they find they are having problems with their eyesight or if they are having more problems with blurred vision or seeing fine detail.

The liver function of the patient should be monitored frequently whilst they are taking Actos. They should see their doctor if they develop any unexplained nausea, vomiting, abdominal pain, tiredness or loss of appetite, dark urine if their skin or the whites of their eyes turn yellow.

If the patient has stopped ovulating because they have had polycystic ovary syndrome, they may start ovulating again and will need to use contraception to prevent pregnancy.  If a woman becomes pregnant or plans to become pregnant they will need to stop taking Actos.
